Three were killed at the scene while another passenger died in hospital at 1.50am from injuries sustained.
Kuala Krai police chief Mazlan Mamat said preliminary investigations showed the accident occurred when a Toyota Hilux veered out of control and hit a Ford Ranger pickup in the opposite lane.
The Hilux, driven by a 37-year-old man with three other passengers, was travelling from Ipoh, Perak, to Machang. The Ford Ranger was driven by a 53-year-old man.
Police will carry out a detailed investigation, he said.
A fire and rescue department spokesman had said earlier that four men and a woman had suffered serious injuries and were sent to Kuala Krai hospital for treatment.
