The Selangor occupational safety and health department said the victim was a new employee tasked with cleaning the factory’s production section.
“The victim was attempting to remove a jerry can that was stuck inside a blow moulding machine during the incident.
“He had opened the machine’s door when a component moved and crushed his head. He was pronounced dead at the scene by medical personnel,” it said in a statement.
The department slapped the employer with warning notices for failing to prepare SOPs for workers regarding the cleaning and maintenance of the factory’s machinery.
It said further investigations are ongoing to identify the cause of the accident and the party responsible, adding that employers who fail to ensure the safety of employees can be punished with a maximum RM500,000 fine, if convicted.
